1. Medical services — scope and limitations

The services offered by this practice are elective, scheduled, and ongoing telemedicine services. They do not constitute urgent or emergency care, nor do they replace a primary care physician.

For medical emergencies, call 911 or go to the nearest emergency room.

The information published on this website is for general informational purposes only and does not constitute personalized medical advice. The doctor-patient relationship is established solely through the formal clinical evaluation process.

2. Acceptance of treatment and outcomes

An initial consultation does not guarantee that treatment will begin. The care plan, if applicable, is determined following an individualized medical evaluation and at the clinical discretion of Dr. Amanda Cabán Herrera.

Treatment results may vary depending on each patient’s clinical condition and individual factors beyond the Practice’s control. Specific results are not guaranteed.

3. Payments, Billing, and Cancellations

Cabán Metabolic Wellness operates on a cash-pay basis. We do not accept health insurance plans as a form of payment.

Payment for the initial consultation is due at the time the appointment is scheduled. Ongoing care, if applicable, is billed monthly in accordance with the terms agreed upon at the start of the service.

No additional charges will be incurred without explicit prior consent. Service prices are clearly disclosed before any transaction.

The cancellation policy in effect at the time of booking applies to all appointments. If you cancel outside the permitted timeframe, a corresponding fee may apply.

Prescription medications are not included in the practice’s fees and are the patient’s financial responsibility.

4. Privacy and Health Information (HIPAA)

As a healthcare provider, Dr. Amanda Cabán Herrera is a covered entity under HIPAA (Health Insurance Portability and Accountability Act). All protected health information (PHI) generated in the course of medical care is subject to HIPAA’s federal protections.

Your rights regarding your medical information, as well as this Practice’s privacy practices, are described in detail in the Notice of Privacy Practices (NPP), available upon request at info@cabanmetabolic.com.

5. Limitation of Liability

La Práctica shall not be liable for any indirect, incidental, or consequential damages arising from the use of, or the inability to use, the services offered, including service interruptions due to technical issues beyond its control.

The Firm’s total liability for any claim shall not exceed the amount paid for the services directly related to such claim.

6. Amendments

La Práctica reserves the right to modify these Terms at any time. Changes will take effect upon publication on the website. Your continued use of the services after such changes are published constitutes your acceptance of them.

7. Governing Law

These Terms and Conditions are governed by the laws of the Commonwealth of Puerto Rico and applicable federal regulations, including HIPAA. Any dispute that cannot be resolved by mutual agreement shall be submitted to the jurisdiction of the competent courts of Puerto Rico.
